MU ALPHA THETA ANDREE AWARD APPLICATION

The Andree Award, named in honor of Mu Alpha Theta founders Richard and
Josephine Andree, is awarded to a student interested in becoming a mathematics
teacher. Mu Alpha Theta will present two $2500 Andree Awards, one to a graduating
high school member and one to a current or former member now in college. The winner
may choose to receive the $2500 as a scholarship to their college or as a cash prize. A
winner accepting cash needs to submit a W9 form so that the award can be reported to
the IRS.
Each chapter may submit one high school senior and one college nomination per
year. The sponsor of the chapter will determine how the chapter's nominees are
selected.

Application must be postmarked by March 1st. PLEASE NOTE ON THE ENVELOPE
"College Andree Award" or "High School Andree Award."

Student Essay: Why I want to be a Mathematics Teacher.
Three letters of recommendation
(High School student: One from club sponsor)
(College student: At least one from professor)
Official High School Transcript
Official College Transcript (If college student)

You will need to make three copies of the Recommendation Form for your three letter
