With whom does the child live?
Karena currently lives with her mother.
Address: #19, Orchid Drive, Jacob Hill Wallerfield.
Is the child at School or other place of education? YES.
Are you proposing any changes within the next two years? YES.
I plan on enrolling Karena in a Private Pre-school close to where I live.
Does the child suffer from any medical disability? NO.
Is the child under local authority or probation officer? NO.

What are the present arrangements for access by you/by the Respondent to each child?
Currently, the arrangement is that I am allowed to see Karena when the Respondent permits me
to. In addition, I can take Karena when I am allowed to, but I must return her on the same day.

Are these arrangements working satisfactorily? NO.

Do you propose any changes? YES.
I would prefer my daughter live with me where I can take her to better schools, protect her
from the advantage she previously suffered and raise her in a more stable environment.
