I found this fluency assessment at

This is instructional and also has assessment. This is research based and is informal. The
research for this one also points to accuracy and prosody as being important for fluency.
It states that both areas need to be assessed. This assessment is one on one because you
need to be able to hear the students read out loud. This assessment also includes a rubric
to use when you are accessing students.
This assessment is to see how fluent a student is. It looks at their automaticityand
prosodywhen reading.
