I have developed
therapeutic relationships
with the clients I have
worked with. Weekly I am
reviewing nursing notes,
consulting with peers, and
staff members in selecting
appropriate tool, and
assessment method.

regulatory standards,

She uses evidenceinformed practice and a
theory-based approach
to care. She asks
questions and clarify the
plan of care, unclear
orders or directions, and
whenever uncertain.

I am able to document care

given, and vital information
in the clients chart, and in
the appropriate places. My
client notes are always
completed in a timely
manner, as to have accurate
up to date information on
my clients status readily
available. I communicate
with the staff, peers, and
clients in a professional
way.

Over the weeks we have all

developed a caring
professional relationship
with each other, and the
nurses at St. Johns. I am
able t o give and accept
constructive criticism. I
share important information
regarding clients, with the
appropriate member of the
team, i.e. nurse, OT or PT.
For example when a clients
pressure mask was not
fitting correctly, I share this
information with the staff
nurse, and together we
brought the mask to the
therapist to get it resized.

An area that a wanted to

gain more experience in
was physical assessments.
Doing them this semester
on a weekly basis has
significantly improved that
skill. I am always going
over my notes on head to
toe assessment, and creating
a systematic way of
completing my assessment.
I always begin my care here
by asking the client about
what goals they have seeing

to develop solutions and
create a positive work
environment. She
provides feedback to
peers and accepts
feedback from peers
and members of the
inter-professional health
care team. She supports
clients rights for selfdetermination and
choice.

Shona develops theory-

are holistic and clientcentered and are
articulated competently

I am aware of my strength
and weaknesses as a
nursing student so within
group settings and play on
my strengths and contribute
in that way to further the
knowledge of the group. I
am always initiating
collaboration with my peers
while performing care. I
want to gain the knowledge
they have and share what I
have gained as well. This
betters me as a nursing
student in the clinical
setting because if I have not
previously performed a
skill, I can gain some
insight, from hearing a
peers firsthand experience.

Weekly I reflect on my
practice, I do so
independently, and with a
peer that I work closely
with. This is a great way to
outline my areas of
strength, and areas that I
need to work on. For
example when I gave my
first subcutaneous injection
on this unit I was extremely
nervous, and didnt perform
as well as I could have.
