Field Trip Permission Slip

Dear Parents/ Guardians,
On July 30, 2015 the Together We Can Academy classroom will be going to the Rosamond Gifford Zoo
located at One Conservation Place, Syracuse, NY 13204. We will be leaving the school at 9:30 a.m. and
returning back here at 3:30 p.m. Transportation will be provided by the local school bus. The costs of
admission are as stated:

Youth 3 5 - $4.00
Adults- $8.00
Please fill out the lower portion and return with the fee (cash only please) of $____ to your
childs classroom no later than July 26, 2015.
Children are to bring a brown bag lunch and a snack with a beverage. All children must wear socks and
sneakers and bring a light jacket. We will be in need of 10 chaperones, please fill out bottom of the
permission slip and have returned no later than July 26, 2015 if you wish to chaperone.
