AFFIDAVIT OF PHYSICAL TRANSFER
I, ERNESTO E. PALADA, of legal age, married, Filipino Citizen and
resident of Paykek, Kapangan, Benguet, Philippines, after having been duly
sworn to in accordance with law hereby depose and state that:
1 I am an owner of a certain Motor Vehicle with Franchise which I sold
to Revelyne P. Chewas as evidenced by a Absolute Deed of Sale of a
Motor Vehicle with Franchise;
2 However, in my Income Tax Return (ITR) I am the one who is paying
the tax for the PUJ which I sold;
3 I will Physically Transfer the PUJ/motor vehicle which I sold to
Revelyne P. Chewas and she will be one to pay the tax due on said
PUJ/motor vehicle;
4 I am executing this Affidavit to attest to the truth of the foregoing
facts and for all legal intents and purposes it may serve.
IN WITNESS HEREOF, I have hereunto set my hand this 22 nd day of
April, 2015 at Baguio City, Philippines.
