Phone intake interviews:
Various houses do phone interviews in different ways. Following are some

pertinent questions that can be helpful when an individual calls in.

Are you clean and sober today?
When was the last time you drank or used?
What is your drug of choice?
Where are you located right now?
What can we do to help you?
Are you familiar with 12 step recovery programs? (AA, NA, CA)
Have you been in a sober living home before? If yes what one and what were

the circumstances of your leaving?

Explain briefly what the sober living home consists of.
Are you currently employed? If not what is your means of support, if any?

Are you willing to go to work immediately?

Do you have medical or court appointments pending? What is your

means of transportation? Who referred you to us?

Tell me something about yourself, age, what are your plans for recovery?
If you feel the person is a candidate for sober living set up an on-site interview

appointment for placement.

On-site intake interviews:
When an individual arrives for an interview first and foremost welcome them

and assure them they are in a safe place.
