Dear Dr. Agbulos:
Greetings!

It is with so much sadness to inform you that I wish to end my contract with the Zamboanga City
Health Office as a Barangay Health Worker and Informal Laboratory Worker of Zambowood
Health Center effective April 13, 2015. This provides your Office with a five- day notice to plan
for my termination.
Being at Zambowood Health Center for five months has given me the chance to learn new things
that may also help me, my family and my new community in the aspect of health and wellness.
My husband and I, together with our kids had decided to move to Davao City to look for a
greener pasture. We would like to take the opportunity to go there so that we would be able to
sustain the education of our children and provide them the best that we could give them.
I am very happy in my experience as a Barangay Health Worker and I will also be thankful for
the support that your Office gave me during my stay. I also wish that you would be able to find
an immediate replacement for the vacancy. I am very much hoping for your consideration and
approval.
