attend our basketball camp this summer.
We will strive each and everyday to not
only improve your basketball skills, but
also make sure that you have an enjoyable
experience. We will focus on the
fundamentals and skill development this
camp. I know you will benefit from being
apart of our camp.
Throughout my time here at York, we have

to act for me, according to their best judgment, in any
emergency requiring any medical attention, and hereby
waive and release the camp from any and all liability for
any injuries or illness incurred while at camp. I have no
knowledge of any medical problem or physical
impairment that would be affected by the above name
campers participation in the camp program, as outlined
in the brochure. The YC athletic trainer services and/ or
any the nearest medical facility is hereby authorized to
render primary medical care to my child during his/her
visit to YC.
