    Mr. Muhammad Jamaluddin, MIO, Employee No.80004428, of IBC Clifton is working in
    RPR section.He was admitted in Liaquat National Hospital on 14-June, 2013, and
    discharged 17-June, 2013. Further 07 days bed rest from 17-June, 2013 to 24-June,
    2013, for one week now he requested to join his duty. (medical certificate is attached)
    It is therefore, requested to please examine and issue the fitness certificate.
