AFFIDAVIT OF CORRECTION
I, CHRISTINE GUERRERO MUNAR, of legal age, married, Filipino
citizen and with residential and postal address at Bangbangolan, San
Fernando City, La Union, Philippines after having been duly sworn to in
accordance with law, hereby depose and state that:
1. I am the daughter of ROMEO P. GUERRERO and ROSEMARIE L.
DULAY;
2. However, in my Certificate of Live Birth issued by the National
Statistics Office (NSO), the name of my mother that was written is
ROSEMARIE D. GUERRERO;
3. The true and correct MAIDEN NAME OF MY MOTHER is
ROSEMARIE L. DULAY and not ROSEMARIE D. GUERRERO,
which name is known to our relatives, friends and in the community;
4. Such error was due to an honest mistake and inadvertence
attributable to the one who prepared my Certificate of Live Birth;
5. I am executing this affidavit of correction to attest to the truth of the
foregoing and to state the fact that the correct maiden name of my
mother is ROSEMARIE L. DULAY and not ROSEMARIE D.
GUERRERO and for whatever legal purposes it may serve.
