2012 ACR Radiation Oncology In-Training Exam (TXIT)

REGISTRATION INFORMATION
Exam Date: March 7, 2013
EXAM REGISTRATION Deadline: January 4, 2013
Registrations are accepted through the online ATPU system or via paper forms (by request). If you have any
questions regarding the registration process, please contact Lynn Anderson at landerson@acr.org or Eugene Nsiah
at ensiah@acr.org.
If your site has a resident who is taking the exam at an alternate site, it is the responsibility of the Program
Coordinator of the home site to contact the Program Director of the alternate site to make exam arrangements.
If contact information for the alternate sites Program Director is needed, please email your request to
intrainingexam@acr.org.
Please note that the ship-to address cannot include a P.O. Box; an alternate mailing address must be provided.

PAYMENT

Deadline: January 25, 2013

The exam price is $275.00 per resident. The registration payment form will be used to submit payment by check,
institutional credit card, or purchase order number. Please RETURN THE PAYMENT FORM with your payment;
and on the check, please reference: Radiation Oncology In-Training Exam, your 4-digit site number, and your facility
name. Checks received without reference information will cause a delay in shipment of scoring information.

Absolutely NO refunds will be granted after the registration deadline of January 11, 2013.

CONSENT FORMS

Deadline: January 25, 2013

Each resident participating in the exam is required to submit a consent form. These forms may be sent at any time
before the above stated deadline. Consent forms can be received by fax to In-Training Coordinator at (703) 716-1283
or by email to intrainingexam@acr.org, or by UPS mail to: ACR TXIT 2013 Exam, Attn: In-Training Coordinator,
