FORMAL LETTER OF SUSPENSION AND DISCIPLINARY ACTION

This letter constitutes a formal notice of suspension disciplinary action being taken as a
result of your unprofessional conduct as a teacher at LIFE Prep. This letter also contains
directives that you are expected to comply with in the future as a teacher at LIFE Prep.
CAUSE FOR DISCIPLINARY ACTION
The cause for disciplinary action is as follows:
Three students reported that on March 10, 2015, you held your hand over the
mouth of a student for several seconds.
DIRECTIVES
You are to adhere to the following directives:
1. You will not put your hand over a students mouth.
2. You will not retaliate against any student or party involved in this investigation.
3. You will keep all student and personnel data regarding this investigation private.
4. You will attend one day of training on effective behavior management practices.
The administration will make the arrangements for your attendance and notify
you in advance. LIFE Prep will pay for the training, supplemental costs, and a
substitute teacher.
These directives will remain in effect unless we inform you in writing that they cease to
apply.

DISCIPLINARY ACTION
This letter of suspension and disciplinary action constitutes a formal written reprimand in
this matter and will become part of your personnel file in District 4035, LIFE Prep.
You are also suspended from teaching for three (3) days without pay beginning
Thursday, April 16, 2015 through Monday, April 20, 2015. You will resume work on
Tuesday, April 21, 2015.
NOTICES
Your failure to comply with the directives contained in this letter, or a reoccurrence of
similar conduct in the future will result in further disciplinary action up to and including
the possibility of termination of your employment from LIFE Prep.
If you have any questions about or do not fully understand anything in this letter or the
standard to which you will be held in the future you are directed to contact me
immediately. If you do not do so, you will be deemed to fully understand the information
and directives in this letter and the standard to which you will be held in the future.
I also caution you that you may not engage in any retaliation toward staff members,
students or others who participated in this investigation. Any retaliation by you will
constitute grounds for disciplinary action.
