Inorder to fastrack the completion of all Regional and Provincial CSC Justification and Appeals
Templates, all Regional and Provincial Personnel Officers are directed to render overtime until
Monday, the 13th of October 2014 and subsequently, submit the same to the DAR CMT-TWG on
or before 12:00 noon of October 14, 2014 through your respective Regional Personnel Officers.
These templates were revised to reflect comprehensive placement and selection process done
by the DAR-CMT. The ecopies of the revised templates attached herewith include the follOWing:

1. Templatll No.1 - Full accounting of all employees and position items under the
DAR Existing Staffing Pattern (ESP);
2. Template No.2 -

. Full accounting of all employees and positions under the approved

DAR Indicative Staffing Pattern (ISP);

3. Template No.3 -

Justification and Appeal re: CSC Field Office Actions on 1,303

Appointments

4. Template No.4 -

Individual Appeal Form

Along with the above-said templates to be prepared on a per region and per province basis, the
Regional Personnel Officers must also bring and submit individual folders of employees with
invalidated appointments together with attached and duly signed Template No. 4 and the
corresponding Personal Data Sheet (PDS) of concerned appointee.
