Objective: to increase student awareness of his/her learning progress by independently

demonstrating critical thinking, problem-solving, and decision-making skills. The purpose of
self-evaluation is to enhance clinical skill development.
Assignment: the student will submit weekly written Self-Assessments by the end of each week
and adddress the following:
1. Describe what you feel you did well in clinic today/this week:
Tuesday: Continuing Care/Recall Patient. 8 week recall, patient was a 1.5 sub
(previously a 3 sub). Patient had significant reduction in pocket depths above 3mm, and
bleeding points. Patient also reduced from a Microscope C to a slight B. He has been
using the Marys recipe 1x/day. This patient is a perio case type IV, his main goal is to
maintain his oral health.
Saturday: Completed and Passed my clinical board exam!!!
