Problem: Sea water out let line from 101.1543A/B & its header found having abnormal high vibration
leading to vibration of complete plate form & adjacent line of 1540 exchanger (line 601.WB 1023), Welding
cracks at the header joint & supports are observed. Repaired by welding a patch at crack location as a
temporary measure.
Possible Solutions: It is recommended to study the piping/support design and implement the corrective
action to reduce the vibration & avoid the fatigue failures.
