Greetings in His Most Precious Name, Jesus Christ!
The College of Business Administration and Accountancy will conduct an educational tour to small-and-medium enterprises
(SMEs) on March 14, 2015. These companies include Asiawide Refreshments Corporation (RC Cola), Zest-O Corporation,
and Manila Cordage Company which are all located in Carmelray Industrial Park, Canlubang, Calamba City, Laguna.
The tour package rate is P2, 400.00 per student. Inclusions are air-conditioned tourist bus transport, services of tour guides
and facilitators, all entrance fees, lunch buffet (Eat-All-You-Can), and travel insurance.
The itinerary is as follows:

5:30 AM Leave THE FISHER VALLEY COLLEGE COMPOUND

9:00 AM Arrive at Pakil, Laguna; E & R Wood Shaving Industry, Pakil Chuch, Danny Galena

with written report of the tour, this will serve as their pre-final examination/class standing grade in different subjects. Faculty in-charged will ensure safety of students and I
therefore agree that The Fisher Valley College will not be liable for any untoward incident and accident that my son/daughter may have.
I am signing my consent below and agree to pay the amount of Php2, 400.00 for the entire tour package.

allowed to join the tour without the signed waiver.
