Borang ini hendaklah di isi sepenuhnya sebelum memulakan keja di tapak binaan oleh Penyelia / Wakil dari Kontraktor Utama dan sentiasa kekal di tapak kerja.Borang yang telah siap diisi mestilah di
simpan sebagai rekod di dalam fail keselamatan projek.

PERSONAL PROTECTIVE EQUIPMENT

ALAT PELINDUNGAN DIRI
Comppulsory to all personnel - Safety shoes and safety helmet.
Diwajibkan kepada semua pekerja - Kasut dan topi keselamatan
Reflectorizedsafety vest - if working on or at the road side or working at night.
Vest pantul cahaya - Kerja di tepi jalanraya atau kerja pada waktu malam.
Others as per job / activity requirement - face,eye,ear,hand protection , etc.
Lain lain menurut tugasan - Muka, mata, telinga,tangan dan lain lain.
All site personnnel possess a valid CIDB Green Card or NIOSH TNB Safety Passport.
Semua pekerja di tapak binaan mempunyai Kad CIDB atau NTSB.
NIGHT WORK

Adequate blinkers and reflectorwere provided
Lampu keselamatan dan pemantul cahaya di sediakan.
All temporary signage are reflectorized
Papan tanda keselamatan memantul cahaya.
Adequate illumination not less than 200 Watts with stand provided.
Pencahayaan dengan lampu 200 Watt serta kaki yang sesuai.
Traffic controller equipped with batton light
Pengawal traffic lengkap dengan lampu isyarat 'batton'.
SITE SAFETY FACILITIES & CONDITION
KEADAAN & KEMUDAHAN TEMPAT KERJA
First aid kit, fire extinguisher are adequately available and serviceable
Peti pertolongan Cemas, Alat Pemadam Api Kecil dalam keadaan baik di tapak kerja.
All rotating or moving or protruding part well protected or guarded
Semua bahagian yang berputar atau terjulur telah di pagari
Trench with depth 1.2m or more provided with ladder and for extended trench the distance
between ladder shall not exceed 30m
Lurah yang berkedalaman lebih 1.2m di sediakan dengan tanga dan jarak antara dua tangga adalah
tidak melebihi 30m.
Trench with depth more 1.5m or more is protected from soil collapse and more than 4m depth
the protection was endorsed by P.E
Lurah yang kedalam melebihi 1.5m di lindungi dari runtuh dan yang kedalam melebihi 4m telah di
endos oleh Profesional Engineer.
The trench was in safe condition and no water logging (Shall be inspected priort to continue job
after raining)
Semua lurah adalagh selamat dan tiada air bertakung (Pemeriksaan perlu dibuat sebelum memulakan
kerja atau selepas hujan)
Number of ladder is adequate, sufficient and 1m above the landing places.
Bilangan tangga mencukupi dan dalam keadaan baik serta mempunyai ketinggian melebihi 1m dari
tapak turun.

No heavy loads located near to the trench and excavated soil was 1m away from edge.
Tiada beban berat yang di letakkan berhampiran lurah serta tanah yang dikorek diletakan 1m jauh dari
tebing.

REMARKS
CATATAN

Work area is well barricaded and safe work access is provided.

Tapak kerja telah dipagari serta laluan selamat di sediakan.
Gas cylinder secured in upright position and properly labeled.
Tabung gas diletak menegak serta telah dilabelkan.
Electrical / Welding generator set equipped with ELCB and grounded during operation
Semua alatan elektrik dan kimpalan di lengkapkan dengan pemutus arus dan dibumikan ketika
operasi.
Tools and other equipment e.g water pump,welding set & electrode holder are good condition
and serviceable.
Semua latan yang terdapat di tapak kerja seperti pam air adalah dalam keadaan baik.
STRUCTURES ERECTION OPERATION
OPERASI PEMASANGAN STRUKTUR
Crane involved in the operatio were already inspected .

All the employee involed has known their function in the opration.

All the electrical conductor nearby the live substation has been earthing or been remove from
the operation area.
Semua konductor elektrik berhampiran substation telah dibumikan atau di pindah keluar dari kawasan
kerja.
All the personnel involved in high level operation were equipped with the adequate safety belt
or body harness.
Semua pekerja yang terlibat dalam operasi yang tinggi dilengkapi dengan talipinggang keselamatan
dan "body harness".
There are no other operation below the high level operation
Tiada operasi lain yang sedang dibuat dibawah operasi yang tinggi.
The base of the structurs were confirm in good condition and safe to be use.
Tapak pemasangan struktur telah dipastikan selamat untuk digunakan.
Weather condition was good for the high level operation
Keadaan cuaca amat sesuai untuk oprasi kerja tinggi.
HOUSEKEEPING

Worksite is in order and tidy and area is safe and secure to general public

Material are safely secured and away from public and traffic
Semua bahan binaan dikawal dan bebas dari orang awam dan lalulintas
Open trench was properly barricaded with sufficient warning signs or flasher placed at specific
location.
Lurah yang terbuka telah dipagari serta dilengkapi dengan papan amaran atau dilengkapi dengan
lampu amaran.
We has confirmed the actions taken in this Safety Daily Checklist and in our judgement the area is safe and work may
