AFFIDAVIT OF WITNESS
, OF LEGAL AGE, Filipino,

Dr. Maria Lioba A. Limpin, of legal age, Filipino, single and with residence and
post office address at 130 San Sebastian St.,Sasmuan,Pampanga after having been
I,

duly sworn to in accordance with law, depose and state:

1. That on July 3, 2014, I was asked by my friend Pacencia A. Tatlonghari if she may go
to our house and, with the assistance of my father, execute her last will and
testament;
2. That as a longtime friend and mahjong buddy, I granted her request and told her to
visit us on July 7, 2014.
3. That on July 7, 2014, Pacencia, together with our other friends,

Francisco Garcia

and Faustino Mercado and another gentleman who was later introduced to me as
LORENZO T. ABAD came to our house;
4. That on our sala, Pacencia orally declares her properties and designate among the
Abad family to whom some properties and amount of money will be given.
5. That while she is doing that, my father types in his laptop the oral dispositions.
6. That after Pacencia is done, my father printed the document which consisted of four
(4) pages in three (3) copies.
7. That my father handed to Pacencia one of the copies in which she affixed her
signature on the left margin of pages 1,2, and 4 and on the bottom of page 3.

Afterwards, my father handed to me the copy signed by Pacencia and was told to
sign at the attestation clause found at page 4 and to also sign at the left hand
margin of pages 1, 2, and 4.
9. That the same instructions were given to Francisco Garcia and Faustino
Mercado and both of them complied with such.
