18 Plaintiff is a state prisoner who is proceeding pro se with
19 an action for damages and other relief concerning alleged civil
20 rights violations. The matter has been referred to the Magistrate
21 Judge pursuant to 28 U.S.C. § 636(b) and Local Rules 72-302 and
22 72-304.
23 Plaintiff’s complaint was filed on April 24, 2006. Plaintiff
24 filed an application to proceed in forma pauperis the same day,
25 but the application was incomplete because the certificate was
26 not completed or signed by an authorized officer, and the
27 application was not accompanied by a certified, original prison
28 trust account statement including the entire six-month period

1 immediately preceding the filing of the complaint as required by


2 28 U.S.C. § 1915(a)(2).
3 In accordance with the above, IT IS HEREBY ORDERED that
4 within thirty days of the date of service of this order,
5 Plaintiff shall submit a completed in forma pauperis application
6 with a signed certification and an original certified copy of his
7 prison trust account statement for the six-month period
8 immediately preceding the filing of the complaint, or, in the
9 alternative, pay the $350.001 filing fee for this action.
10 Plaintiff’s failure to comply with this order will result in a
11 recommendation that this action be dismissed.
