The member was diabetic, had chronic heart failure and coronary
artery disease. And, as I quickly found out, she had very little
money.
She said the health plans case manager was working to help her
obtain glucometer testing supplies. Which was good, but she
needed food. Good nutrition is so important for staying in control
of her medical conditions. As it was, she said she had been living
on cereal, peanut butter and jelly and noodles.
I called a community services agency and left a message for
assistance and submitted a case management referral. I wasnt
sure when someone would get back to her, so I used a break in
my schedule later that day to go grocery shopping.
The member was shocked when I returned with eggs, bread, milk,
some meats, cheese, vegetables and soups. In opening the
refrigerator to put things away, I found a pint of milk, some
condiments and a half-full jar of apple sauce. The freezer was
completely empty.
Before I left, the community services agency called saying they
would see her that afternoon to fill out emergency paperwork for
supplies from the food pantry.
Todays visit really made a difference in this members day!
