    The following note came in from a field care manager for the
    health plan.
    I have had the pleasure of working with Nichole and wish to
    compliment her on her professionalism and terrific rapport with
    my members. I have been with her on several home visits and
    was impressed by her knowledge and expertise.
    She was able to identify and troubleshoot a very important health
    issue for one of my members and quite possibly saved her life.
    This member has a very complex history. After an exam by
    Nichole, the member was identified as having severe carotid
    artery blockages. She facilitated an urgent screening and
    ultrasound. The member had over 85 percent blockage on the
    right side and 75 percent on the left. This member will be in
    surgery this week!
    I look forward to working with Nichole in the future and only wish I
    could have her assistance for all of my members.
