AFFIDAVIT OF EXPLANATION
I, ELEAZAR R. FRASCO, Filipino, of legal age, married, and with
present address at 621 Basak, Mandaue City, Cebu, Philippines, under
oath, do hereby depose and say:
1. That on 06 January 2012, I was issued a Philippine Passport by
the Department of Foreign Affairs (DFA) Cebu bearing Passport
No. EB4386223, which I kept in my files for safekeeping.
2. That very recently however, I discovered that my said
passport was already dilapidated (with its front page
damaged) thus, is no longer fit for use.
3. That the damage to the said passport was by mere
inadvertence and was never intentional.
4. This affidavit is executed to establish the veracity of the
foregoing facts in support of my request for re-issuance of
passport by DFA- Cebu before its supposed date of expiration
(January 5, 2017) and for whatever legal purpose this may
serve.
