AFFIDAVIT OF MUTILATION
I, ALVIN CASTRONUEVO CABADA, Filipino, of legal age, single,
and with present address at 177 Plaridel St., Mandaue City, Philippines,
under oath, do hereby depose and say:
1. That I was previously a resident of 124-A Old Road Sagkahan,
Tacloban City and was employed at EXPERT GLOBAL
SOLUTION, with business address at Pawing, Palo, Leyte as a
Customer Service Representative;
2. That however, due to the devastation brought about by the
onslaught of the Super Typhoon Yolanda (Haiyan) in Palo,
Leyte last November 8, 2013, my employment was preterminated as the business operation of EXPERT GLOBAL
SOLUTION is no longer practicable;
3. That due to the foregoing reasons, I could not secure a
Certificate of Employment and Certificate of Clearance from
EXPERT GLOBAL SOLUTION especially so that until today, it
has not resumed business operation;
4. This affidavit is executed to establish the veracity of the
foregoing facts in support of the pre-employment
requirements of CONVERGYS with business address at Skyrise
Building Asiatown I.T. Park, Lahug, Cebu City and for whatever
legal purpose this may serve.
IN WITNESS WHEREOF, I have hereunto set my hand this 10 th
