WE, RUBEN J. LAO and CLAIRE B. LAO, Filipinos, both of legal ages,
and with residence at #56 Rusellia St., NTHA Subdivision, Nasipit, Talamban,
Cebu City, Philippines, under oath, hereby depose and say:

1. That we are the biological parents of REI JIRO B. LAO, who was born
on July 3, 2004 at Cebu City, Philippines;
2. That the said Rei Jiro B. Lao is a minor and will travel together with us
as tourist;
3. That we, as parents are the one responsible in providing for his
financial support and shelter during the entire duration of the travel
and will guarantee that he will not become a public charge or liability
in the said country;
4. This Affidavit is executed in order to establish the veracity of the
foregoing facts and for whatever legal purpose it may serve.
