PETITION
Undersigned Petitioner, unto this Honorable Court, most
respectfully states that:
1.0 She is of legal age, born on 21 June 1990, single,
Filipino and residing at ____________________, Philippines.
2.0 She is a member of the Philippine Bar, and was
formally admitted to the practice of law on 2015 with Roll of
Attorneys No. ______
3.0 She is a member in good standing, having been
listed as a Member of the Integrated Bar of the Philippines,
____________ Chapter under _______________
4.0 She has paid her 2015 Professional Tax under
Receipt No. ____________, a copy of which is attached herein as
Annex A.
5.0 As a lawyer, she holds office at EJS Building,
Veterans Avenue Extension, Zamboanga City.
6.0 Pursuant to Rule III, Section 1(4) of the Rules of
Notarial Practice of 2004, the following additional
requirements are attached herein as follows:

a. Certification and Clearance from the Office of the

Bar Confidant, Supreme Court, Manila, as
Annexes C and D;
b. Certification from the Integrated Bar of the
Philippines National Office, signed by the
National Treasurer and Director for Bar
Discipline, as Annex E;
c. Certificate of Good Moral Character, signed by
the two (2) executive officers of the local chapter,
as Annex F
d. Three (3) pieces photograph duly signed by
Petitioner and three (3) specimens of his official
signature and true impressions of his notarial
seal, as Annexes g, H and I, respectively;
e. Official Receipt Nos. _______________ in the
amount of Php 2,020.00, as proof of payment of
the required filing fee for this Petition, as Annex
J hereof;
7.0 Petitioner was previously appointed as a Notary
Public for and in the City of Zamboanga for the year
2012-2013 and the same expired on December 31, 2013.
8.0 He has submitted his Notarial Report from 2012 to
December 2013, as evidenced by a Certification issued by
the Clerk of Court, Regional Trial Court, Zamboanga City,
a copy of which is attached herein as Annex K.
9.0 He has neither applied for nor been appointed as a
Notary Public in any other place.
PRAYER
WHEREFORE, it is most respectfully prayed that
Petitioner be APPOINTED as a Notary Public for and in the

City of Zamboanga, Philippines, effective January

and inclusive until December 31, 2015.
