The purpose of this letter is to both summarize our recent evaluation and to formally caution warn
you regarding your excessive tardiness to work. As we discussed, you have been tardy to work
nine (9) times since March 11, 2015. This level of tardiness is unacceptable in that as it poses an
undue hardship demonstrates grievance on your co-workers and affects the productivity of the
entire department.
In the future, you must adhere to the following directives:
1)
2)
3)
4)

Report TO WORK on time and GO HOME on time daily as scheduled (10:00am-7:00pm).

Sign-in and/or clock-in on time and as directed by our Superior.
Report all absences or tardiness in advance to your Superior.
No overtime filing on you department unless requested to do so and this has been approved.

Reducing your tardiness is critical and is a requirement for your continued employment. Failure
to reduce minimize your tardiness, failure to non-adherence to any of the above directives, and/or
any other misconduct may result in the docking withholding of your pay, suspension, or the
recommendation for the termination of your employment.
Please be guided accordingly.
