    SOURCES OF FUND : LGU- MUNICIPAL HEALTH OFFICE LANTAPAN BUK.
    RATIONALE:
    In line of the medicade goal of the department of health by year 2013-2016 program that
    86% of the have sanitary toilet facility constructed and 96% of potable water supply for household in
    every LGU, the municipality of lantapan , bukidnon in the program review it shows to uor data in year
    2012 is one of the lowest(%) percentage of the sanitary toilet contructed only 68% and diarrheal case is
    4th of morbidity cases in the municipalty.

    Objectives:

    1.To reduce the unmet needs especially to identified 4ps


    2.To meet the percentage medicade goal by year 2013-2016
    3.To reduce diarrheal case

    Budgetary Requirements:
    For the period of 3 years , 4,163pcs , toilet bowl porceline 2, 414,540 pesos/3=1388 per year
    805, 040 pcs.

    EVALUATION/RECOMMENDATIONS
    Program and project of LGU-MHO in line of the medicade goal of the department of health will
    be promoted and done. Hence that the amount of eight hundred five thousand and forty pesos for year be
    approved and released.
