1. ln Bursuit of developing policies, Etarrdarde and guidelines on youth formation programs,

the Depafiment of Education through an Unnumbered Memorandum *igned by Ussc. Rivera,
dated May 13, 2415, conducts an evaluation of schoolclubs in public schoole in the Philippines.
2.

ln consonance with the above mentioned memorandum, all school heads are advised to
accomplish Form { (Evaluation Form forthe School Head) as printed on the back page.

3.

Accomplished Form No. 1 should be submitted to this office through Mr. Macaraig, EPS
1/LRMDC ln-Charge on or before July 21,2015.
