The Complication of Urethral Diverticulum

Abstract
Urethral diverticulum is a rare urology condition, moreover with stone formation inside the
divertiulum. Maisuria et al report the incidence of anterior urethral diverticulum in South
Gujarat, India is 1,2% and diverticular stone complication occurred in 4%-10% patient. Anterior
urethral diverticulum with stone with scrotal abscess complication is a very rare and very few
cases were reported. We report a case of 49-year-old man who presented with scrotal swelling
and scrotal pain for about 20 days. Abdominopelvic CT showed peno-scrotal urethral projection
stone sized 3 cm x4 cm x 4 cm with fluid collection in scrotum. On exploration we found
bulbous urethral diverticulum with stone and scrotal abscess. We performed diverticulectomy
with stone removal and debridement of the scrotum. End to end anastomosis of the urethra was
carried out 16 days after first surgery waiting for the granulation in the open wound appeared.
Patient regained normal voiding function with uroflowmetry result Q max 21,1 mL/s, Qave 13,9
mL/s, voided volume 301 mL and there is no contrast extravastion from retrograde
urethrography evaluation performed in the 6th week postoperatively.
