MEMORANDUM FOR ALL CONSUMER-DIRECTED SERVICES VENDORS

Money Follows the Person

This memorandum is to notify Consumer-Directed Service (CDS) Vendors of a change in the fax
number to which all the information related to Money Follows the Person (MFP), including the Request
for Supplemental Transition Funds, should be sent. The new fax number is 573-526-5047.
The Request for Supplemental Transition Funds form has been updated to include the new fax number,
the participants community address and date of the participants transition. The form can be located at:
http://health.mo.gov/seniors/hcbs/appsforms.php.
