Subject: Payment of Annual Ceiling for Medicines on monthly basis to Banks

Retired Employees

Kindly refer to HO.

Karachi instruction Cir No 159/2009 dated 18-12-09 under which procedure for
reimbursement of retired employee’s medical bills has been changed. According to the
said circular the employees / officers separated under GHS / VHS have also been
included in the medical scheme effective from 01-01-10.

In this connection, I want to bring to your kind notice that I was also separated under
GHS and was relieved from Bank Service on 20-01-1998. I was at that time posted as
Bank Manager at NBP Sakot Branch code 1379 District Rawalpindi.

I also bring this information to your kind notice that at the time of separation from Bank
Service, my service in the bank was above 25 years as I was appointed and joined my
service on 12-08-1972. Certain papers regarding Bank Service are being attached with
this application for your kind perusal.

In the light of above mentioned facts, I may kindly be sanctioned medical ceiling
introduced in the above referred circular. So that I could support my family smoothly.
