REQUIREMENTS FOR ADMISSION TO

Medicine Program
1. Applicants must be holder of Bachelor's degree in Science or is expected to finish the course before the
start of school year.
2. Within or in addition to the Bachelor's degree, the applicant must have the following academic
credits:
SUBJECT
a. Mathematics

UNITS

(preferably Algebra, Trigonometry, and Statistics)


b. Chemistry

c.1. Zoology

c.2. Botany

d. Physics

e. Social Sciences

3. Applicants must submit certificate true copy of NMAT score of 40.


4. Must submit a certification of good moral character and a recommendation letter from the school last
attended.
5. Original Transcript of Records
6. Birth certificate issued by NSO
7. Photocopy of diploma
8. General weighted average and school ranking
9. I.D. picture 2x2 5 pcs. White background
10. Once requirements #1-9 have been submitted and evaluated, applicant must fill up completely and
accurately one (1) information sheet available at the office of the Dean, Program of Medicine.
MEDICAL ADMISSION TEST (MAT)
This test is administered to students who seek admission to the Medical Program of the University and who
satisfactorily meet the Admission Requirement of the University and the College.
Procedure for TAT/NAT/MAT/LSQT
1. The Applicants shall first go to the corresponding college for assessment of grades and requirements.
2. The Dean of College shall make the endorsement for qualified applicants to take the test needed for the
course/program.
The Testing Center shall process the application forms and set the test schedules of the qualified applicants.
