Welcome Letter from the Director: Cheryl Upper 2015-2016

Welcome to CDMs peanut-free facility! The first day of classes is: Monday, September
14th, 2015. We have an exciting school year planned for you. This booklet is filled with
important information about the operation of our school. If you have any questions or
concerns, please speak to one of administrators in the office at your earliest
convenience:

WAIVER: Every parent or guardian is required to sign our waiver. If you registered online,
you have done this already by checking the required box. If you registered in any other
way, you should have signed a hard copy waiver. You can download a waiver form from
our website>registration tab> how to register then click waiver. If you are unsure,
please ask at the reception desk.
COMMUNICATION: We request that you keep all communications for the school year as
you receive them from our office. Information regarding our fee schedule, holidays and
cancelled classes are on the following pages and will also be posted on our Google
calendar right on our home page: go to www.cdmdance.com. Our primary method of
correspondence is by email, so please be sure to provide us with your most updated
email address. You can do this by logging into your online account. Please keep the
square indicating, YES, I agree to receive emails checked on your account. We ask
that you check your email at least twice per week. If you do not have email access,
please notify us so we can find an alternate method of communication between CDM
and your family. You will also be expected to read the important information that is
posted on the CDM Information board in the school lobby. At times, we will also send
home printed information. Regularly utilize our website to find out the latest information
and released schedules, as well as access your online account. Please refresh your
screen for latest website updates. Much of the information is located under Existing
Students.
LOBBY, VIEWING AND WIFI: CDM offers a comfortable lobby as a place to wait for your
child during lessons. The code to the door at the bottom of the stairs is Schlage 1-5-1-5.
This lock helps keep studio secure from strangers. Please avoid removing shoes and
leaving them on the floor at the bottom of the stairs where they become tripping
hazards. Use the shoe racks provided.We give you the opportunity to view all classes by
either a viewing window OR a video monitor. We also provide seating for those waiting to
pick up their child or students waiting for their next class. Please clean up after yourself if
you choose to snack in the lobby. Only water or tightly covered drinks and dry NonPeanut snacks are allowed. We encourage you to enjoy the shopping in the plaza during

when the studio is at a peak busy time to avoid unnecessary congestion. The television
may be watched provided it does not disturb others. The remote control is kept at the
reception desk. Just ask us. We offer free wi-fi at CDM. Our network is danceCDM and
password is lovedance.
WAITING IN THE LOBBY: We ask that you respect the activities of others and the classes in
session. There is no running permitted in the hallways and no acrobatics in the lobby.
Small children waiting with parents must be closely monitored. If you use our books
magazines and toys, we would appreciate it if you would return them to the magazine
rack or toy area. Your courtesy with this will be greatly appreciated.
BEFORE AND AFTER CLASSES: Students are requested to arrive to the studio with adequate
time to prepare for class, parking, changing, putting up hair up properly, restroom, etc.
For Parents picking up young children: please be available in the waiting room to receive
your child when dismissed from class. Although we have their safety in mind, we do not
have extra staff on duty to care for young children between or after classes. All Parents:
please make sure to tell your child they must not leave the lobby area or go through the
lobby door until they are picked up. Also, let us know if you will be having a visitor with
you within the premises.
We welcome your comments and suggestions regarding any of our policies or
procedures at any time. Feel free to send us an email or talk to someone in the office so
we can address your concerns. If any parent/student wishes a private conference
concerning your individual dance goals or financial concerns, please do not hesitate to
ask for an appointment with us so we may assist with your personal needs. We are happy
to work with you to ensure that your school year will be one of pleasure and growth. The
entire faculty and staff wish you a fantastic year of dance!

STUDIO POLICIES & CLASS ETIQUETTE


Please note the following guidelines regarding all classes:

1. DRESS CODE: We request that students carefully observe our dress code outlined in this
booklet. Buns are required for all ballet classes. (Video: How to do a bun). If your family is
having difficulty for any reason in following the dress code, please let us know. We will do
our best to help. The dress code is included in this booklet as well as on our website under
Existing Students

2. ARRIVING LATE: For your safety and to receive full benefit from your class, any student
arriving more than 15 minutes late may not be admitted to class or may be asked to
observe. Teachers have the right to make this decision. Students should enter quietly and
not disrupt the class or distract other students in class. Please arrange for a make-up class
if you expect to be very late. This can be done online or by calling the office.

3. LEAVING THE CLASSROOM: All students are encouraged not to leave the room once
class has begun. Please use the restrooms prior to class and send your child with a filled
water bottle into the classroom. Very young children will be treated sensitively as the
need arises. Preschool children may need a parents attention from time to time for
various reasons including bathroom or behavior issues. Please do not leave the waiting
room when very young children (18 mos.-4 yrs.) are in class without informing the office or
having another parent responsible during your absence. We make every effort to have
all children feel safe and cared for in a loving and inspiring learning environment.

5. CLASS DISRUPTION: School age children and teens:Please practice respect. Students
are not to be socializing in a disruptive manner during the class with other students.
Please come early or stay late to enjoy friends in the waiting area. Honor your teacher
and fellow students by giving full attention and co-operation. Parents, you will be
advised if we feel any student is not well adjusted to the classroom environment for any
reason over time.

6. FEELING UNWELL OR SUSTAINING INJURY IN-CLASS: During class if a student complains


of being ill, or sustains an injury and cannot participate, the instructor will use their best
judgment and either have them sit out or send them to the office were the parents will
be notified if not on the premises.Please review the waiver you have signed

7. COMING TO CLASS IF SICK OR INJURED: We encourage that injured students or


students with a non-contagious illness observe their classes. The parents must notify the
office if a student will be observing class. A note, email or phone call will suffice. See our
policies on our website on how to make up a class.

8. VISITORS TO CLASS; Out of town guests may request to visit at any time throughout the
year. They are welcome to watch the class with prior notice. Otherwise, no one else is
allowed to sit and watch during regular class time.

9. BEHAVIOUR IN CLASS: Students should be respectful to the teacher and their


peers. Attentive listening, following directions and a positive personality is
encouraged. Hanging on the ballet barre and touching the divider wall is not
permitted. Please raise your hand if you would like to ask the teacher a question.
Talking with peers during class is not encouraged unless participating in a group
or partner task.

Dance class is educational and fun, however, an important part of class work is
developing an attitude of discipline and respect for teachers and fellow
students. We trust that following our classroom etiquette will provide an
environment whereby classes atConservatory of Dance & Musicwill be a
pleasure for all concerned.

Tuition Fees
(Price per 3 months, approx 12 classes, 3 terms per year)
A $25 non-refundable registration fee per family is required upon registration.
Discounts and Promo codes will be applied to your account by our administration staff
manually. When calculating tuition fees, payment must be made for the higher priced
class/es before applying discounts. Promotions are not transferable and may not be
combined between siblings.

General Payment Information

$35/hour & up

Your payments will be paying for the entire school year of 36 weeks of classes held over
September 14th to June 11th 2016 (recital day which is the last day of the season). There
are an average of 12 classes per term making 36 classes in the year. There are holidays
that may fall on Mondays: Thanksgiving, Family Day, Victoria Day, (as noted in our list of
holidays). If you are enrolling for a recreational class on a Monday and are not part of
the Carte Blanche Program you are permitted to take 3 make-up classes at or below
current level. Classes may be in another dance style. Please choose these classes at time
of enrollment and let us know in the office. We suggest taking these classes within Term 1
or 2 when recital choreography is not the priority. You are not paying for scheduled CDM
vacations and Holidays. Please see our published calendar in this booklet or the

homepage on our website cdmdance.com. Our $25 Registration Fee is due with
September tuition. New students joining after September will have their class charges
pro-rated from the time they join at the rate posted on the current posted fee schedule.
The calendar in each month will vary depending upon which day you have your class.
Some months you may have 2 or 3 classes, others 4 or 5. Everyone will have had 36
classes over the ten months. Recital and Photo days are counted in your tuition, as we
do not charge a separate recital participation fee or photo shoot fee for photographers.
There are also rehearsals at the end of each photo shoot and onstage before recital.
Those students joining in Term 3 will not be refunded for classes missed for Photo Day or
trying on costumes day. There will be no refund for those who choose not to take part in
recital or photos. No exceptions.

Late Payment Fees:Outstanding payments are subject to a $10 late charge after
14 days from the initial payment due date. After 30 days from the initial payment
date, another $15 will be charged. After 45 days, CDM reserves the right to
prohibit students with outstanding fees from attending classes until full payment is
received.
NSF cheques are subject to a $25 charge.
If your credit card information has changed, please notify the office before the
next payment deadline.

Other possible fees that may incur include (but are not limited to): Guest artist
workshops, weekend workshops, RAD exam fees, competition and recital
photography, recital DVD, dancewear, team warm-up suit, travel costs to
competitions, hotels.
Missed Classes/Cancelled classes:
Please let us know if you will not be able to attend a class (via email or telephone). We
will pass this information on to the instructor. Missed classes must be made up within the
same term of your absence. No credit will be given for missed classes. You can schedule
your make-up class online through your own online account. No credit is given for
student absences. Students who observe religious or cultural holidays not noted on our
calendar that require a student to miss a class can choose a suitable make-up class with
the studio manager or director. No credit will be given on the account.CDM reserves the
right to cancel any class with a low enrolment of less than 4 students. We will give notice
of a cancelled class and will do our best to offer an alternate option.Classes cancelled
due to unexpected inclement weather will not be rescheduled outside of class time.
However, classes can be made up in any existing class at or below the students level.
Make up classes will be made up if a teacher is unexpectedly ill and a suitable substitute
could not be found in time, or for any other unplanned cancellation (Carte Blanche
classes and extra competitive rehearsals are the exception). If you make a payment
late, please include your late fee as it will be billed to your account. If you make your
payments by mail, be certain to allow adequate time for mail delivery.

Withdrawal Policies / Refunds:


We have a very fair policy regarding our payment for tuition and subsequent assessment
of late fees.Any student who withdraws from a class before November 3, 2014 will be
charged a withdrawal fee of $50 and will be refunded a pro-rated amount of the Term 1
Payment paid in September. Post-dated cheques will be returned in full. Any student who
withdraws from a class after November 3, 2014 and before January 3rd, 2015 will be
refunded only payments for terms 2 and 3 only. No tuition refunds will be issued after
May 16th, 2016. Absences do not confirm that a student has withdrawn. All withdraws
must be submitted via email. This email will determine official withdrawal date. No
refunds will be issued after June1st 2016.

The quality of our facility and programs depends upon a system of mutual respect and
cooperation. We understand that anyone may make a late payment from time to time,
but please be respectful of our late fee policy and when applicable, graciously include
your late fee in your next payment.

Competition Entry Fees Refund Policy- After January 4th, 2016 refunds will not be
granted.CDM will not refund for injuries occurring due to an extra-curricular activity.
However, each case will be reviewed individually. MP: Please note that although the
schedule is current, class times, dates, instructors and studio allocation ARE SUBJECT TO
CHANGE to accommodate low enrolment, staffing scheduling and otherfactors. Any
withdrawals that families must make due to a schedule change that does not work for
your family, we will suggest an alternate class for you.

Code of Ethics

We the dancers, faculty and staff of Conservatory of Dance & Music, are a vibrant and
caring community of people who have a high regard for each other, the art of dance,
all art forms, and the respectability and value of the work we do. All who participate are
requested to demonstrate the willingness to be held accountable to uphold the values
and ethics we declare as the fundamental guidelines stated below. In all of our
endeavors to study and perform as an expression of our love of dance and the arts, we
show respect to the parents, our peers, the staff and faculty of Conservatory of Dance &
Music and all of the wider dance community, including our other schools and studios in
the industry including those at competitions. We consider it a privilege to participate in a
worldwide community for the performing arts exemplifying the values of integrity,
character, leadership, responsibility and professionalism. We are all expected to honor
CDMs Code of Ethics. Failure to do so may result in a meeting with a member of staff,
studio manager or director. CDM reserves the right to end contracts with any one
refusing to honour the code of Ethics.
At Conservatory of Dance & Music, We promise to conduct our selves with:

To conduct oneself with honesty, courtesy, and respect. To
express consideration for ones fellow dancers, educators, parents, staff and
competitors. To be a person who leads by example and honors commitments. To
be a person other people can count on.

goals and ideals. By setting an example through ones actions, speech and
thoughts. To inspire and serve the people of your class, team, or community
through full participation in all dance related activities.

RESPONSIBILITY: To respect and adhere to the policies and procedures of the


studio or the organizers of any conference, workshop, or competition. To be a
person who interacts with peers and authority figures in a mature manner. To
question authority in a respectful manner. To speak of any issue or grievance to
the person who is able to resolve any concern. To be willing to receive coaching
and direction when deemed appropriate.

CHARACTER: To be a person who represents high levels of personal integrity,


leadership, and responsibility for oneself and others. To be a person who has the
courage and willingness to face issues and concerns with maturity and dignity.
To avoid gossip, criticism, or denigrating comments for any member of the dance
community, parent, or peers.

PROFESSIONALISM: To respect the dance profession and profession in the fine arts
and uphold our values and standards of conduct. To support and encourage
one another. To voice opinions and initiate change that will elevate the dance
community. To recognize oneself as a role model at all times. To love the study
and performance of dance as a universal art form.

INCLEMENT WEATHER CANCELLATIONS & POWER OUTAGES


On days where weather becomes an issue for safe travel to the school, we will observe
the local news and generally follow TDSB cancellations. Where possible we will try to
notify classes by email or phone. Please check our websites home page calendar to see
if we have cancelled classes for the day. We will also update our outgoing phone
message to update any class changes due to inclement weather. If we experience a
power outage we will send an email out as soon as possible.
We will hold classes except under exceptionally dangerous driving conditions. When
there is a delayed school opening, all morning classes will be held. Use your own
discretion regarding attendance if the driving is difficult from your area.
If a class is cancelled due to these uncontrollable circumstances, make-up classes will
not be scheduled outside of their regular time (exam coaching classes are an
exception). Cancelled classes may be made up by attending another class at or below
the students level. Classes may be made up in another dance form. No credit will be
given for missed classes; however, students are always welcome to take make-up classes
for any missed class.

Dress Code
Although we encourage students to express themselves, it is important for students to
abide by our dress code. We have a dress code policy for the following reasons:

A neat and tidy appearance sets an attitude of attentiveness and respect for
students, teachers, and the art of dance.

Teachers must be able to see the dancer's body outline clearly in order to make
proper corrections on posture, alignment, etc.

Please do not wear underwear under tights where possible (younger students) as
they show beneath the leotard and tights.

A bun is required for all ballet classes. Hair must be properly secured and tied up
off of the face and neck for all styles of dance. Wisps or stray hairs should be
sprayed flat with hairspray. If you have difficulty making a bun, please attend our
information/welcome meeting in September to learn how or ask us how in the

No jewelry unless previously discussed with instructor.

Gum chewing is not allowed due to danger of choking and to maintain respect
for the class, your peers and teachers.

No loose fitting pants, blue jeans or daywear shirts, swimwear or bikini tops (they
are not dance attire).

No costumes are allowed in class unless, a dress up day is announced by the


individual teacher.

CDM Apparel
Help unify and promote our school with CDM logo Apparel. We have t-shirts for kids and
adults in a variety of cuts and sizes, as well as team jackets, suits and jogging pants. They
are great to wear outside of the studio, to school, sports events, before and after class.

Girls: Any classical-style leotard (tank or camisole). Black
preferred but any colour permitted. Students may be asked to
purchase a leotard of a chosen style and colour as chosen by
their instructor if an exam will be taken. For the exam, a
matching chiffon skirt may be necessary. CDM has some skirts
available to borrow for mock exam and exam day provided
your leotard matches our skirts (burgundy, purple, mint green,
ask for updated colours). Bodywrappers Tights A31 or A80
BAT($22). Pink leather or canvas, full or split-soled ballet
