Introduction
BMA oral health survey in the year 2001 found that caries prevalence on 6-yr-old children was
78.6% and caries prevalence in 12-yr-old was 66.0% and DMFT was 2.0. Dental Health Division,
Department of Health, BMA is responsible for oral health services in Bangkok including oral health
promotion and prevention. Oral health promotion and prevention activities for BMA students have been
implemented gradually started from tooth brushing after lunch with fluoride toothpaste, pit and fissure
sealant, school health volunteer, tooth brushing drill using disclosing tablet, oral health promoting school,
and fluoridated school milk. Dental Health Division creates a network and works together with schools,
parents, and children. Schools conduct their own activities to achieve oral health promoting school
concept. 3 major activities of health promoting school are knowledge management in oral health education,
healthy diet and avoid unhealthy snacks, and tooth brushing after lunch.
Aim
1. To pay tribute to the 50th birthday anniversary of Her Royal Highness Princess Maha Chakkri
Sirindhorn.
2. To decrease dental caries and promote healthy oral hygiene among BMA students.
Method
Milk fluoridation project was first introduced in 10 BMA schools in 2001. Government and BMA
provide school milk to every BMA students. President of The Borrow Foundation, UK and Milk fluoridation
project committees discussed about Milk fluoridation project for caries prevention among BMA students
with Governor of BMA in 2005.
An academic meeting on the topic of fluoride and caries prevention for BMA school directors and
responsible teachers working on the project is organized once a year. Schools notify and convey
knowledge to parents and students. Number of schools participated in milk fluoridation project increased
gradually. Schools administrate daily fluoridated milk intake.
Dental health division summarizes fluoridated milk management yearly report about problems,
comments, and management from BMA schools. Milk fluoridation project workgroup finds solution for the
problems.
Results
Milk fluoridation project is expanded to 300,000 students in 431 all BMA primary schools of 50
districts in Bangkok since 2007. Every year students from kindergarten to grade 6 in BMAschools receive
one bag of 200 cc fluoridated milk contained 0.5 mgF. for 260 days.
Discussion and Conclusion
Adding fluoride into existing school milk is practical for fluoride supplementation. Schools find
the project is suitable and acceptable because students receive both healthy diet and fluoride
supplement at one time. Fluoridated school milk blends nicely with oral health promoting school concept
(healthy diet).

Project assessment is a 5-yr.serial cross-sectional study using oral health examination and
questionnaires.
1. Data collection at year 1(baseline in 2007), 3(2009), and 5(2011).
2. Study sites are: 4 sites in Bangkok (study gr.) and 4 sites in Bangkok vicinity (control gr.).

3. Sample size is: 300 students in each 4 age gr. (4-5, 6-7, 8-9, and 11-12).
4. WHO Survey form is use to collect: Dental caries experience (in surface), Oral hygiene
status, and Fluorosis.
5. Data on oral health behavior is collected by 3 questionnaires: ask parents and students about
oral hygiene habit, behavior, and dental care.
Fluoridated milk production and quality control.
U Royal Chilada Project provides academic and technical support on training of fluoridated milk
production and quality control, and equipment replacement in case of emergency.
U Food and Drug Administration, Ministry of Public Health inspects every dairy plant and authorizes
yearly fluoridated milk production.
U Department of Health and Food and Drug Administration, Ministry of Public Health visits every
dairy plant before fluoridated milk production and monitor in between production.
U Department of Health, Ministry of Public Health examined fluoride concentration in milk sample
once a month, found that fluoride concentration was in standard range (2.5+0.3 ppm.). The result was
the same as reported from dairy plants.
U BMA schools sent some milk samples to Microbiology subdivision, Health Laboratory Division,
Department of Health, BMA. for microbiology tests: found that milk were in good standard.
Fluoridated milk management and Health promotion in BMA schools
Department of Education and District Offices purchased 200 bags of pasteurized fluoridated
milk (2 school semesters of 100 school days) and 60 UHT fluoridated milk (2 school vacations of
30 days) for each student.
Department of Education and Department of Health conducted a meeting to provide
information to school principals and teachers about fluoridated milk management in schools,
fluoride in dentistry and precaution.
Management in school.
o Children under 3 yr.
o Children with kidney disease.
o Children under 6 yr shouldnt drink more than 1 pack per day.
o Children over 6 yr are able to drink 1-2 packs/d.
o Control appropriate amount of milk consumption.
o Do not take any food, drink or tooth brushing after drink Fmilk for 30 minutes to
maintain F concentration in oral cavity.
o Student who drinks Fmilk shouldnt take another F supplement but are able to use
topical F or Ftoothpaste.
o Remaining milk should be kept in refrigerator for tomorrow consumption.
Schools
o Provide information to parents and ask for permission from parents to join the project.
o Provide leaflet of fluoridated milk consumption on vacation.
o Encourage and monitor appropriate milk drinking habit both in schools and at home.
o Cooperate with dairy plants in milk distribution and quality control.
o Milk storage and distribution in schools.
o Conduct other health promoting activities, for example: health exhibition in school,
tooth brushing after lunch, proper school lunch, and dental health promotion week.
Schools and Dental Health Division conduct health promoting activities, such as;
o Provide oral health care instruction and related information.
o Students oral health examination.
o Dental pit and fissure sealant on first permanent molars for students grade 1 and 2.
o School health volunteers training.
o Tooth brushing drills, proper health behavior practices.
