Stage 3 extends deeper into tissue below the skin. Stage 4 is the most severe where damage extends to muscle and bone.     Pressuresores(bedsores)areaninjurytotheskinandunderlying
    tissue.Theycanrangefrommildreddeningoftheskintosevere
    tissuedamageandsometimesinfectionthatextendsinto
    muscleandbone.Pressuresoresaredescribedinfourstages:
    Stage1soresarenotopenwounds.Theskinmaybepainful,butit
    hasnobreaksortears.Theskinappearsreddenedanddoesnot
    blanch(losecolorbrieflywhenyoupressyourfingeronitandthen
    removeyourfinger).Inadarkskinnedperson,theareamayappear
    tobeadifferentcolorthanthesurroundingskin,butitmaynotlook
    red.Skintemperatureisoftenwarmer.Andthestage1sorecan
    feeleitherfirmerorsofterthantheareaaroundit.

    Atstage2,theskinbreaksopen,wearsaway,orformsanulcer,
    whichisusuallytenderandpainful.Thesoreexpandsintodeeper
    layersoftheskin.Itcanlooklikeascrape(abrasion),blister,ora
    shallowcraterintheskin.Sometimesthisstagelookslikeablister
    filledwithclearfluid.Atthisstage,someskinmaybedamaged
    beyondrepairormaydie.
    Duringstage3,thesoregetsworseandextendsintothetissue
    beneaththeskin,formingasmallcrater.Fatmayshowinthesore,
    butnotmuscle,tendon,orbone.
    Atstage4,thepressuresoreisverydeep,reachingintomuscle
    andboneandcausingextensivedamage.Damagetodeeper
    tissues,tendons,andjointsmayoccur.
    Instages3and4theremaybelittleornopainduetosignificant
    tissuedamage.Seriouscomplications,suchasinfectionofthe
    bone(osteomyelitis)orblood(sepsis),canoccurifpressuresores
    progress.
    Sometimesapressuresoredoesnotfitintooneofthesestages.
    Insomecases,adeeppressuresoreissuspectedbutcannotbe
    confirmed.Whenthereisn'tanopenwoundbutthetissuesbeneath
    thesurfacehavebeendamaged,thesoreiscalledadeeptissue
    injury(DTI).Theareaofskinmaylookpurpleordarkred,orthere
    maybeabloodfilledblister.Ifyouoryourdoctorsuspecta
    pressuresore,theareaistreatedasthoughapressuresorehas
    formed.
    Therearealsopressuresoresthatare"unstageable,"meaningthat
    thestageisnotclear.Inthesecases,thebaseofthesoreis
    coveredbyathicklayerofothertissueandpusthatmaybeyellow,
    gray,green,brown,orblack.Thedoctorcannotseethebaseofthe
    soretodeterminethestage.
