Re: Disqualification of Maxer team in Bataan General hospital

Dear Sir Dennis,


In response to the situation happened last May 11, 2015 in Bataan General hospital,
As a Senior Biomedical Engineer of Endoscopy Department, I was the one leading
the team in office preparing the machines and instruments. I was given a copy of
list of things needed to bring in the demonstration. As like the other
demonstrations, I included all the machines and instruments normally used in all of
our demonstrations to other clients. As normally we do, we prepare the machines
needed a day before the given schedule. Maxer tower is a complicated medical
equipment to handle as we need to setup different machines and its accessories
down to each hand instruments needed to use during the operation. On the list that
given to me, I complied all the specifications written on it even the accessories that
can be bought from the local market. I wasnt aware that we need to provide all the
accessories on that certain client because other clients are not requiring the
accessories that can be bought locally. The only problem that Mr. Jordan Diciembre
and Mr. Alex Josep encountered is that they dont have the clips that can be used by
surgeons to ligate the affected artery on liver or other parts of the digestive system
during the operation. Some surgeons used it, some are dont as there are other
ways to ligate the affected artery. Ligating clips are not a maxer product as we need
to outsource it to other local distributor. I met some local distributor of the clips
during the conventions and try to befriend them for as to have a close relationship
and at the same time, I can contact them if ever we need the clips. I made a
purchase to them previously and use the clips as part of our demos. We spent all
the clips that I bought and upon asking for another set of clips, they said that it is
not available that is why we dont have a stock of clips.
As we go back to Bataan General Hospital incident, they disqualified us because we
dont have enough clips that they can be use during their operation on that day.
Machines and instruments are complete. Only the ligating clips are the issue.
The topic of incident report was brought up to me as soon as the committee
checked our equipment on-site and upon reporting of Mr. Jordan Diciembre to me at
the morning of the same day.
