ABSTRACT
Background. Diabetus mellitus (DM) is an endocrine disorder of increased blood glucose levels by
abnormalities in the beta cells of the pancreas gland Langerhans islands. Type 2 diabetes mellitus is the most
common type which is characterized by hyperglycemia. It may causes a negative impact on the patient, such as
impaired wound healing and increased susceptibility of infection. Therefore, patient with diabetes needed some
consideration before, during, and after treatment. Purpose. The aims of this case report is to recite procedure
fibroma management of patient with type 2 diabetes mellitus, either before, during, and after dental treatment.
Case Report. 56-year-old female patient came to the hospital dental poly Rajavit, Thailand with complaints
contained buccal gingival enlargement in the upper right area of the canines and first premolars were hurt when
chewing and bleed while brushing. The patient had a history of type 2 diabetic, but do not have an allergy to the
drug. Complete blood examination showed normal CBC results, but at 213 mg% FBS. Intraoral examination
showed that the patient had poor oral hygiene, calculus, and periodontitis. Case Management. Patient
concerned to control diabetes. After normal blood sugar levels, patient was given a scaling and oral hygiene
education. On the second visit, the mass removed by excision and extraction of upper right canine and first
premolar, then irrigated and sutured. Postoperatively the patient was given antibiotics. Conclusions.
Management for patients with type 2 diabetes mellitus were done by evaluating blood glucose levels before,
during, and after dental treatment.

Pendahuluan
Diabetes Melitus (DM) merupakan suatu kelainan
endokrin berupa peningkatan kadar glukosa darah
oleh adanya kelainan pada sel beta pulau
langerhans kelenjar pankreas.1 Kelainan ini ditandai
penurunan absolut atau relatif dari kadar insulin
yang
dihasilkan
oleh
pankreas. 2
DM
diklasifikasikan menjadi DM tipe 1, tipe 2,
gestasional, dan tipe lain. Jenis DM yang paling
banyak dijumpai, khususnya di Indonesia adalah
DM tipe 2. Hampir 90% dari kasus DM yang ada
merupakan DM tipe 2.3
DM tipe 2 atau non-insulin dependent DM terjadi
karena kelainan dalam pengikatan insulin dengan
reseptor.
Kelainan
ini
disebabkan
oleh
berkurangnya jumlah tempat reseptor pada
membran sel yang selnya responsif terhadap
insulin, sehingga terjadi penggabungan abnormal
antara kompleks reseptor insulin dengan sistem
transport glukosa. Tidak memadai dan beredarnya
insulin
dalam
darah
menyebabkan
ketidakmampuan darah mempertahankan kadar
glukosa dalam darah.4 Keberadaan glukosa dalam
darah yang berlebih dapat berdampak buruk pada
sesorang, seperti gagalnya proses penyembuhan
infeksi serta menyebabkan manifestasi di berbagai
organ termasuk rongga mulut.2 Pasien dengan DM
memiliki tanda dalam rongga mulutnya seperti
xerostomia dan bau mulut yang khas. Xerostomia
atau penurunan jumlah sekresi saliva yang
berlangsung lama dapat menimbulkan berbagai
komplikasi rongga mulut, seperti gingivitis
