3. Nature of Event: TA Set withdrawn due to primary super heater tube leakage.
4. Observations :

1) Turbine trip first, 2) Turbine Emergency trip,3) Master trip Relay 86GT,4) TLR operated.
5. Analysis- BTL at primary S/H due to ash erosion.
6. Works carried out by maintenance section

Primary S/H top bank coil no. 68 lifted, in tube no. 19 (main failure) due to flue gas erosion
fish mouth opening observed. Secondary damage in coil no. 67 critical .
Total HP joints-30
Total X-ray-30 done

7. Works carried out for the restoration by operation :

8. Recommendations to avoid reoccurrence :

a) Short term
b) Long term

Eroded tubes replaced. Shields in near by coils checked, attended.


1) Based on the historical records, further coils will be lifted and repairs /

rectification of maximum such coils will be done during next O/H,2) Moreover complete
replacement of primary S/H coils programmed in 2011.
