To Qualify You Must Have:

Live in Lee County
No health insurance
Household living up to
and including 250% of
Federal Poverty Guidelines
You Must Bring With You
Lee County Photo ID,
Social Security Card &
Proof of income for entire
household (Tax Return,
Pay Stubs, W-2)

Follow up breast care will be funded by PFBCC for qualifying clients at no cost to you.
