I have read the FECS Laboratory Safety Manual (available in every Science and Engineering
Laboratory) and the Online Orientation Training topics.
I understood all the contents delivered and will follow all the rules and regulations while using the
laboratory here at the university. I REALIZE that many accidents are caused by NEGLIGENCE
and I WILL behave in a manner that will ensure the health and safety of everyone in the laboratory
at all times.
I will wear proper attire to the lab at all times.

I will only access/enter/utilize the lab facilities during assigned lab hours and technicians &
lecturers must be informed

I agree to follow these safety rules and any other safety procedures specified by the Faculty. If I
neglect these safety rules I understand I may be removed from the laboratory portion of the
course and the Faculty will have the right to take disciplinary action on me.

semester for record keeping.
