Reverence Home Hospice, Grand Blanc MI 8/2014-Present
Case manager for a twenty plus patient case load. As a case manager, job
duties included managing hospice and palliative care patients from admit to
death. Managed all emergencies and took call weekly. Worked with multiple
disciplines to ensure care for both the patient and the family. Educated
patients and families on disease process and end of life care and decisions.

Direct Care Nurse: As a direct care nurse, job duties included the delegation
and supervision of patient care staff, patient safety and education, along
with education of the family, and teamwork with multiple disciplines in the
care of the patient. Focus of care was on the care of Hospice patients and
their families.

Nurse: As a circulating nurse, job duties included direct patient care and
safety of patients under anesthesia, along with the sterility of the case and
operating room. Worked as a member of the Neuro and the General Surgery
team.
