Welcome to Concord

Mission
This semester we have been given the opportunity to experience many aspects of Public Health Nursing, a sub
category of Community-Oriented Nursing. This type of nursing focuses on the healthcare of communities and
populations rather than that of individuals, families, or groups. The goal of Public Health Nursing is to prevent
disease and illness and to promote health throughout the community and the many populations it withholds. To
guide our practice we focus on the public health ethic of doing “the greatest good for the greatest number of
people.” (Stanhope & Lancaster, 2006, pg. 4)

The goal of the community assessment project is for a varying number of groups to collect information on
specified communities, and then use this information to identify health disparities associated with the population
assigned. Our group is focusing on the zip code 28027 which primarily encompasses Concord, NC. For this
project the population of focus for our group is End of Life.
Upon completion of the wiki, our mission is to complete the following required components:
• 3 problems identified & prioritized with rationale with a clear connection to the windshield survey,
key informants, epidemiology and other reputable sources.
• 2 measurable objectives for top priority problem, including a description of applicability of Healthy
People 2010 (possibly 2020) to this issue.
• 2 proposed interventions for addressing priority problem, with inclusion of evidence from the
literature as rational for the proposed interventions.
The first step in beginning this process is to provide a detailed description of the areas and its population. To do
so we will have incorporated a wide range of information including the community’s history, demographics,
culture, health related statistics, and epidemiology. With the utilization and integration of this information our
group hopes to identify a primary health disparity for the end of life population of Concord, NC.
