AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This agreement, made and entered into this _____ day of September, 2015, in
Urbiztondo, Pangasinan by and between:
The Urbiztondo Integrated School, represented by Freya R. Tamondong, Ed.D., School
Principal IV and Schools Division Office I of Pangasinan, represented by Atty. Efren Joe
Millora, Legal Officer, Mr. Jojo Jose, Education Program Supervisor and Dr. Oliver
Gutierrez, Education Program Supervisor-SGOD, hereinafter referred to as the FIRST
PARTY;
-andAvelino Soriano, canteen owner and Juvy Pascual, canteen owner hereinafter referred
to as the SECOND PARTY;
WITNESSETH
Whereas, the FIRST PARTY, through Madam Maria Celia Junio-Fernandez, Schools
Division Superintendent, Atty. Efren Joe Millora, demanded the SECOND PARTY for
two (2) consecutive days on 20 July 21 July, 2015 respectively to vacate the
premises;
Whereas, the FIRST PARTY, through Dr. Oliver Gutierrez and Mr. Jojo Jose demanded
for the third time on July 22, 2015 the SECOND PARTY to cease its operation and
finally leave the premises;
Whereas, the SECOND PARTY, requested for an extension from the FIRST PARTY to
continue the operation of the canteens located within the school premises until March
31, 2016 to enable the former to realize the return of investments.
NOW THEREFORE, both parties further consent that the canteens owned by the
SECOND PARTY will continue its operation and cease from the same on March 31,
2016 to give way to a new canteen solely operated and maintained by the FIRST
PARTY through the Urbiztondo Integrated School.
IN WITNESS WHEREOF, parties herein affixed their signatures on the date and place
above written.
