Purpose:
This tuition waiver program is intended to showcase the educational efforts of underrepresented
students at SLCC, and to help them attain their educational goals by rewarding them with a one-
semester tuition waiver. The selection committee will base its decision on a review of the
applicants ePortfolios and the required short essay.

Application Requirements (Complete this form and attach all of the following):
Submit the Free Application for Federal Student Aid (FAFSA) available online at
www.fafsa.gov and have the results sent to Salt Lake Community College (Code: 005220).
A one to two-page essay on your experience creating your SLCC ePortfolio. You may reflect
on the technology, the process of building the ePortfolio, what the ePortfolio signifies to you,
its place in your educational plans, your impressions of its impact on your own thinking, or
any other aspect of the ePortfolio.

Eligibility Requirements:
Have completed 15 or more General Education credits at Salt Lake Community College by the
end of Fall Semester, 2015.
Utah Resident and Citizen or eligible non-citizen of the United States.
Enroll for at least nine credit hours per semester.
Maintain Satisfactory Academic Progress.**
Have a GPA of 3.0 or higher by the end of the Fall semester of the application year. (Verified
by selection committee)
Agree to present your ePortfolio at an awards ceremony in January.
Agree to allow the ePortfolio office to show your ePortfolio to others for marketing and/or
educational purposes.

Preference will be given to individuals whose test scores allow placement into college level (1000
level and above) in English and Mathematics and who meet any of the following criteria:

**Failure to maintain satisfactory progress may result in termination of a tuition waiver for the term. Tuition can
be waived only once per semester. A student may have only one tuition waiver scholarship per academic year.
Tuition waivers cover resident tuition up to 18 credit hours per term. Students must pay their own fees.

ePortfolio Judging Criteria



Winning ePortfolios will display all of the following:

The ePortfolio follows the standard SLCC outline as specified in the Student ePortfolio
Handbook, and therefore contains navigation pages for Welcome, Goals and Outcomes,
Coursework, Outside the Classroom, and Resume.

The Welcome page is, in fact, welcoming and informative. It should set the context of the
ePortfolio, describe what it contains, and introduce the ePortfolios author to the visitor.

The Goals and Outcomes page contains the students goalseither as a list or as a
narrative paragraph. The page also contains links from SLCCs College-wide learning
outcomes to artifacts in the ePortfolio.

The Coursework page has links to all of the General Education course work the student
has completed so far. Remember, students must have taken at least 15 credits of General
Education to be eligible for this award.

Reflection that accompanies the students General Education assignments.


Winning ePortfolios will also display one or more of the following:

Exceptional performance on the signature assignments displayed in the ePortfolio.

Creative and effective use of multi-media, which might include digital photographs, audio,
or video files.

Especially strong connections made between course assignments. This would be


demonstrated in the students reflective writing, and s/he would show how knowledge or
skills learned in one class helped him/her learn in another class or perform well on other
assignments.

A personal statement on the about me section of the Welcome page or on the Goals and
Outcomes page in which the ePortfolio author articulates the relevance of his/her
General Education course work in advancing their own journey toward life-long learning.

Exceptional reflection throughout the ePortfolio.
