Situation:
The implementation of a hybrid training program for the staff of Bright Minds Youth
Development in Jacksonville, Florida will be evaluated. The other training, such as CPR, will be given
in the face-to-face setting and the handbook component for new staff orientation will be given in
the online format. The online format will be implemented and evaluated in the Logic model chart
below. The Inputs in the model are in the format of the Modules. The outputs are describing the
activities for the participants learning. The participation in the assessments are listed under the
outputs as well are listed by title. The outcomes that will be impacted are measured in short,
medium, and long term goals.
