    SKIM AKREDITASI MAKMAL MALAYSIA (SAMM) Revised Scope Of
    Accreditation Sought
    With reference to the above matter and your letter ref no : JSM/AD-700/01/04/939 dated 11
    January 2015 is referred. We would like to revised Scope of Accreditation Sought for
    Kumpulan Ikram Sdn Bhd ( Southern) which have been registered previously. Our
    application received on 5 January 2015 and will be valid for 2 years from 11 th February
    2015 until 11 th February 2017.
    Hereby we are please to attach our new application form for SAMM accreditation. Also to
    attach our old application form for SAMM accreditation for your reference.
