on site, however I was informed they
are kept in headquarters.

some contractors dust masks were left

Nobody should be allowed on site without


a hard hat on. This should be included in
the site induction training.
All workers should undergo a toolbox talk
about PPE

Scaffolds
19. Some guard rails were missing and
some toe boards were missing. (see
picture 2, 3) (WAH Regs)

immediately and scaffold inspected

20. Some boards were in a poor


condition and there were some lying on
scaffolding causing a possible trip
hazard (see picture 15). Some boards
extended beyond transoms causing
head bump hazards (see picture 16).

21. A section of scaffolding has been


altered and standards removed with
diagonal bracing extended onto the top
of an adjacent wall. There are no
warning signs on this scaffold and no
records of inspections. (see picture

This section of scaffolding should be


closed and warning signs put on it.
Scaffold should be inspected immediately
and made safe. Get structural engineer to
inspect condition of scaffold and wall.

22. There were men working on a


trestle with the ends of the boards
resting on blockwork. Working height
was approx. 1.2m above floor level and
above scaffold height on the outside.

Workers should be shown method


statements and advised of safe system of
work, advise of WAH methods in weekly
toolbox talk. Provide scaffold tower to
carry out work.

23. There were men climbing through


scaffold handrails and guards onto step
ladders at a lower height. (WAH Regs)

24. Two board bracket used to support


mini skip. This places the skip outside
the hoarding below with no protection
fans on the hoarding to protect the
public or workers below. There is also
no netting on the skip to stop debris

with hoarding around base to keep public

height above landing place. This ladder
was also not secured or tied to prevent

despite men working on the roof. It
seems to have been removed to allow
work to carry on at below roof level.
