Reservation Form
To help us plan the conference effectively, we discourage walk-in participants
especially due to the limited slots in the hands-on ICT training workshops. Kindly fill in
this form if you are interested to attend the conference but have not yet deposited
payment. Please note that the early bird fee of P4,000 only applies up to Sept. 30.
Regular registration fee is P4,500 from Oct 1 to 20. Once you have deposited
payment, you will need to complete Registration Form 2 so that you can be counted
among the participants. Thank you and we hope to see you in Bacolod!
