Thank you for your interest in Chicago Prairie Tennis Club. Our membership fees are as follows:
Adult Single Membership: $35.00
Family Membership:
$45.00*
*The Family Membership consists of persons residing in the same household consisting of a spouse/significant other,
children up to 18 yrs of age and/or students up to age 25. All Junior Development participants must have a CPTC Family
Membership.
Lake Meadows Residents: Free for qualified residents**
Single $20.00 for non-qualified residents***
Family $30 for non-qualified families***
**Lake Meadows residents must provide proof of residency, such as drivers license or State I.D. with the current name
and address of the person(s) applying for Membership. The application must be approved by the Lake Meadows
management office. If application is denied, the regular Chicago Prairie Tennis Club membership rates will apply. Family
membership consists of persons residing in the same household consisting of a spouse/significant other, children up to 18
yrs of age and/or students up to age 25. All family names listed on the application must be included on the lease at the
Lake Meadows Management office in order to qualify for the free single/family membership.
***Non-qualified memberships for Lake Meadows residents include residents of the 601 building and townhouse
residents. As mentioned above, proof of residency is required.
Chicago Prairie Tennis Club also offers Permanent Court Time for an additional
$50 for Single Memberships and $100 for Family Memberships
This is a one time annual fee for court reservations at the Lake Meadows Tennis Center from May through September of
the current membership year. This fee applies to adults and juniors. Standard court fees are $6 per hour and will be
collected from all persons who do not have permanent court time. Court time is free to approved Lake Meadows
residents.
Guest membership fees at the Lake Meadow Tennis Center are $6 per visit, per person
up to 3 visits. After that regular membership rates apply minus the pre-paid guest fees.

Chicago Prairie Tennis Club Member Benefits

CPTC is the oldest African American Tennis club in the nation, celebrating 101 years of love, service and advantage to
the African American Tennis Community. We are a non-profit organization, promoting the sport of tennis to both juniors
and adults. Some of the benefits of membership include: participation in our junior development programs; free tennis
play and refreshments at our membership Open House that will be held on Sunday, May 19, 2013 and again at our end
of the season event, for members only, in September. Both events are held at the Lake Meadows Tennis Center located
at 32nd and Ellis. You will receive a membership newsletter and are invited to attend our general membership meetings
held six times per year. CPTC sponsors two USTA sanctioned tournaments, the Clarence Walker Jr. Memorial
Tournament for junior tennis players in May and the Chris Scott Memorial Tournament for adults in June. Your

membership enables you to participate in the 68 annual Tri-City Tournament, consisting of tennis competition between

Chicago, Cleveland and Detroit. This year the tournament will be held here in Chicago, August 30 September 2
over the Labor Day holiday weekend.
We also host an annual New Years Eve Day tennis social, plus a host of tennis instruction programs and special events.
Membership fees are $35 for individuals and $45 for families. Lake Meadows residents are entitled to free or discounted
membership rates. Our membership year begins June 1st of the current year and ends May 31st of the following year.
You must have a CPTC membership to participate in a number of events. If you would like more information about

CPTC LAKE MEADOWS TENNIS CENTER WAIVER AND RELEASE

I hereby apply for membership in the Chicago Prairie Tennis Club (CPTC). I agree to abide by the rules and regulations
of CPTC. It is understood by joining this Club, I become eligible to participate in all Club activities and am entitled to all
benefits of membership, including Club social activities.
While participating in events at the Lake Meadows Tennis Center, I assume all risks, regardless of severity, associated
with or resulting from my participation including, but not limited to, injuries, damages and losses of any type or nature, all
such risks to myself being known and appreciated by me. I hereby release, waive, indemnify, defend and hold harmless
Chicago Prairie Tennis Club, Lake Meadows Tennis Center, XS Tennis, LA/Ballys Total Fitness, Lake Meadows Phase I
A Limited Partnership, Lake Meadows Phase II A Limited Partnership, Draper and Kramer, Incorporated, and their
principals, directors, officers, directors, partners, agents, contractors and employees from any and all claims, causes of
action, damages and losses (including, without limitation, death or personal injury) resulting from or in any way related to
my participation at the Lake Meadows Tennis Center. Further, I hereby grant full permission to any and all of the
foregoing to use any photographs, recordings or any other record of this event for any legitimate purpose, including
advertising.
I HAVE READ AND FULLY UNDERSTAND THE WAIVER AND RELEASE OF ALL CLAIMS SET FORTH ABOVE.
